Health Requirements for School Attendance 2019-2020

A physical exam is required for pre-school, Kindergarten, 6th and 9th grade. This exam must be documented on the State of Illinois Certificate of Child Health Examination Form. The exam must include a lead screening for pre-school and Kindergarten, diabetes screening, doctor’s signature with phone number, and a health history with parent’s signature. A sports physical will not meet the requirement for these grade levels. However, the school physical will work for participation in sports. Please check with your health care provider to see if your child is up to date on his/her immunizations. You may also visit for the immunization schedules.

A dental exam is required for Kindergarten, 2nd, and 6th grade. An eye exam performed by an optometrist is required for Kindergarten.

Please contact Katie Main, district school nurse, at with any questions you may have regarding health requirements. All the forms needed can be found under the documents section of the school website under the health information/nurse folder.
