Returning Students & New Students
Parents MUST COMPLETE this year's HEALTH FORM for EACH STUDENT to proceed further.
Parents MUST COMPLETE the following DISTRICT PERMISSION FORM for EACH enrolled student.
Parents with Students entering 6th grade - 12th grade in sports MUST COMPLETE the SPORTS AND INSURANCE FORM
Parents with Students entering 6th grade - 12th grade MUST COMPLETE the CHROMEBOOK AGREEMENT FORM
MILITARY CONSENT is required for Grades 10th - 12th Students
