State Physical and Immunization Requirements
For Public Schools Pre-K
1. A physical dated the year of enrollment in preschool
2. All required immunizations for that child's age (see below)
Kindergarten
1. A physical dated the year of enrollment in kindergarten
(a new one,even if done in preschool the previous year)
2. All required immunizations for a 5 year old student
(see below)
Fifth
1. A physical
dated the year of enrollment into SIXTH grade
Fifth
grade no longer needs an exam. An exam dated AFTER
Aug.15,
2006
for incoming SIXTH graders is acceptable for the '08-'09
year
only.
2. No immunization requirements for this grade unless child is
behind on their immunizations.
Ninth
1. A physical dated the year of enrollment in 9th grade.
2. A Tetanus booster
Any New Students or Out of State Transfers
1. A physical on a State of Illinois Physical Exam Form or
equivalent dated from their latest required year.
(Pre-K, K, 5th, or 9th)
2.New students have 30 days to provide proof of these
requirements;otherwise they will be excluded from school.
Immunization Requirements
1. (1) MMR by Pre-K and (2 total) by Kindergarten
2. (4) HIB by Pre-K
3. (3) Polio by Pre-K and (4 total) polio by Kindergarten
4. (4) DTAP by Pre-K and (5 total) by Kindergarten
5. (3) Hepatitis B by Pre-K
6. (1) Varicella (Chicken Pox) by Pre-K (Started in 2000)
7. (1) TD Booster by 9th grade (no more than 10 years
between TD's)
**ALL STUDENTS THAT NEED TO FULFILL THESE
REQUIREMENTS WILL HAVE UNTIL OCT. 15TH TO DO SO.
** ALL PHYSICALS MUST BE ON AN IDPH PHYSICAL FORM
For students needing a sports physical exam, it is acceptable to use the State of Illinois Department of Human Services Certificate of Child Health Form, however the sports physical IS NOT acceptable as a general physical exam that is required for school attendance.
